These forms are only to be used by veterinarians who will be consulting directly with Dr. Hunthausen about a client's pet.

Click on the appropriate form(s) below for your consultation.

When the form appears in your browser, you can:

a) Hit the print icon on your browser to print the forms.  They will need to be faxed or mailed to us.
                       OR
b) The text can be copied from your browser, inserted into a Word for Windows document, filled out and emailed back to us. 

Veterinarian Form

Be sure to download, fill out and return this form with the history forms.  It has info about the service, fees, and information to be filled out by the veterinarian working up the case.
